Its appearing logical theoretically. With a Perfect sense. But in practical situation how each patient behaves will not be according to research papers or text book descriptions. So I am also waiting to hear from my friends who are dealing with cases on day to day basis

Bottom line he is questioning some fundamental logics. For ex; Remdisvir in late stages is actually being debated by many of us. The way he was talking seems that he should be also a well informed person.

Hi Aparna attha, just want to share my views regarding yesterday's discussion.
I have worked in covid icu for nearly six months, managing all age groups and found not much benefit from the two drugs which we were using , remedesvir and toclizumab.
Moreover, it is not the viral replication that we are more worried but the cytokine storm that is actually causing harm. So what I have noticed is that people with good immunity are equally prone for its complications.
Moreover we have many leukemia kids and those suffering from nephrotic syndromes and so on who are on immunosuppresants.
These kids ,I am surprised to see that they just have URI symptoms. None of them had even pneumonia.
So target drug should suppress our immunity or to say regulate our immunity.
This is what we are following here.

Yes attha, according to WHO remedesvir is not recommended now.

But every doctor now demands pts for bringing remdesvir and families are running pillar to the post
Why so much importance to an useless drug. If not for business interest.

Yes , because there are no trials for other drugs,
Remedesvir was actually for some other virus ebv, and also HCV but futile.
Just extrapolated its use here.

@Monica, In Mallik Paruchuri video he says IL6 pathway has no role. But in my observation so far , I have seen il6 surge as the indicator of a worst prognosis (mortality). What is your experience!?

IL 6 alone , I am not sure.
In our institute, they are not doing routinely, but we have other available markers like d dimer , fibrinogen, pt inr and the very simple test complete blood count.
lymphopenia is a bad prognostic marker.
Radiological diagnosis is more correlating for prognostic purposes ,like we have CoRajxnds score and it's easily available

We have Aparna daughter sushruta getting ready for next generation along with Monica. Having few doctors in family really helps in the time of crisis. Fortunately I have 2more doctors in my in laws place Dr Sekhar and Aditya from young generations
